Terms and conditions

In addition to reaffirming my full acceptance of the terms and conditions of the INTERVENT Privacy Statement and Participant Services Agreement (https://www.myintervent.com/Home/ServiceAgreement):

I authorize and direct INTERVENT to charge my VISA, MasterCard, American Express or Discover card for the payment and program choice that I have selected above. I understand that when INTERVENT sends its transaction notice(s) to my credit card company such notice(s) shall constitute a valid directive that payment of the above amount(s) is (are) fully due and authorized by me. When my credit card company accepts any such transaction by charging my account, such acceptance and recording will serve as my only receipt of such payment.

If I have selected a monthly charge plan, I affirm that I am entering a binding contractual agreement with INTERVENT (or its assignee or licensee) for a period of 12 months from this transaction date. INTERVENT (or its assignee or licensee) has the right to collect any unmade payments for the period of this contract. I understand that if any draft for payment is not honored by my credit card for any reason, I am responsible for that payment plus a service fee of $25.00. This service fee is in addition to any service fee my bank may apply.

In the event I elect to terminate or withdraw from an INTERVENT program because INTERVENT has not fulfilled its responsibilities as outlined in the INTERVENT Pledge, I may receive a refund of any monies that I paid directly to INTERVENT for unused services (refund based on a non-refundable fee of $149.00 and the number of remaining to be scheduled, but unused, coaching appointments) provided:
1. I notify INTERVENT in writing (marked ATTENTION: Chief Operations Officer) of the specific responsibility that has not been fulfilled by INTERVENT;
and
2. INTERVENT is unable to rectify this in a manner satisfactory to me within seven days of receiving my written notification.
